Neighborhood Health Center

In 1986, the Northwest Buffalo Community Center responded to the growing need in the City of Buffalo’s Black Rock/Riverside community by submitting and receiving a Public Health Service 330 Grant from the Health Resources and Service Administration (HRSA)/Bureau of Primary Health Care (BPHC). This grant was awarded to establish Northwest Buffalo Community Health Care Center (NWBCHCC) as a private, not-for-profit, Article 28 Diagnostic and Treatment Center for the purpose of providing primary healthcare services. Thereafter, NWBCHCC became the region’s first established Federally Qualified Health Center (FQHC), seeing its first patient on August 3, 1987.
 
Growing from its modest beginnings in 1987 to present day, NWBCHCC became Neighborhood Health Center (NHC) in 2011 and now acts as a premier safety-net primary care provider to more than 26,000 unique patients across Western New York. Services included in NHC’s suite of primary and integrated care specialties include: Internal Medicine/Family Medicine (IM/FM); Pediatrics; Obstetrics & Gynecology (OB-GYN); Dental health; Pharmacy services; Behavioral Health, Nutritional services; and Podiatry.
 
As an inclusive and welcoming healthcare organization, NHC is dedicated to treating the whole you, regardless of ability to pay. NHC accepts all patients, participates in most insurance plans and specifically focuses on the uninsured and under-served populations. The Health Center utilizes a sliding fee scale structure for uninsured patients and incorporates the Federal Government’s poverty level matrix and the patient’s household income to determine the co-payment for services. 
 
As a multi-site enterprise, NHC currently has four brick and mortar offices (including a mobile office unit) in its network of primary healthcare centers including: Neighborhood Health Center | Blasdell (4233 Lake Avenue, Blasdell, NY 14219); Neighborhood Health Center | Mattina (300 Niagara Street, Buffalo, NY 14201); Neighborhood Health Center | Northwest (155 Lawn Avenue, Buffalo, NY 14207); and Neighborhood Health Center | Southtowns (OB-GYN services only office at 151 Elmview Avenue, Buffalo, NY 14075).
